Our Emergency Department is open 24 hours a day, 7 days a week. If you are experiencing a medical emergency, call our emergency line immediately or come directly to our facility.

- Loss of consciousness or severe confusion

- Stroke symptoms (facial drooping, arm weakness, speech difficulty)

Patient Rights & Responsibilities
Understanding your rights and responsibilities helps create a partnership in your healthcare journey.
Your Rights
- Be treated with respect, regardless of gender, race, religion, ethnicity, disability, economic circumstances or allegations of crimes.
- Relevant information in a language and manner the patient understands. Information made available to patients shall include diagnosis, testing, treatment to be used, the risk of treatment, side effects of medication and the cost medical care.
- Receive urgent, immediate and sufficient interventions and care in the event of emergency.
- Know health care team and right to develop a collaborative plan to prevent recurrence of medical problems.
- Decline care, choose or change Health Care Provider and shall take responsibility for what happens if planned treatment is rejected.
- Decline or consent to participation in medical research, experimental procedures or clinical trials.
- Be treated in clean, safe and secure healthcare environment.
- Privacy and confidentiality of medical records.
- Timely access to detailed and accurate medical records and available service.
- Transparent billing and full disclosure of any costs, including recommended treatment plans.
Your Responsibilities
- Give correct and complete information about current symptoms, including pain and medication.
- Give information about medical and health history.
- Request information about range of services available including the identity, skills and credentials of professionals and care givers.
- Ask questions if treatment being given or planned treatment by a Health Provider is not understood or part to be played in the plan is not understood.
- Follow recommended treatment plan agreed to by Health Provider.
- Report dissatisfaction through appropriate channel provided by the Health Provider. Provide sufficient details of dissatisfaction. If dissatisfied with outcomes, report to appropriate regulatory authority.
- Keep appointments or notify Health Providers within reasonable time frame if there is a need to cancel visit.
- Follow health facility rules regarding smoking, noise and use of electrical and electronic equipment as well as limit the use of mobile devises within health facility.
- Pay for care including emergency treatment.
- Treat service provider staff with courtesy and respect, free from discrimination and harassment.
- Comply with directives, instructions and advice of attending professionals.
- Comply with operational rules and regulations of the facility for the safety and security of all.
- Respect the rights of other patients.
Patient Safety
Your safety is our highest priority. That's why Well Clinic has a comprehensive program dedicated to patient safety. Our goal is to eliminate hospital-borne infections and medical errors. We encourage all patients and visitors to take an active role in their personal safety.
Infection Prevention
Infection prevention is everyone's responsibility. Here are the things we do to encourage hygiene and keep our patients safe:
- Hand hygiene is the simplest and most effective way to prevent hospital-borne infections. Antibacterial soap is available in every bathroom.
- Masking and hand gel stations are available throughout Well Clinic facility.
- All care providers (e.g., nurses, doctors, lab scientists) are required to wash their hands before and after they care for you.
- All care providers are required to wear proper personal protective equipment (e.g., gloves, mask, gown) for the duties they are performing.
- Speak up! You have the right to request that your care provider wash his or her hands before caring for you.
Patient Identification
Patient identification is crucial for ensuring safety and accuracy. These are the steps we take to ensure we're always treating the correct patient:
- You will be given a wristband upon admission with the following information: name, date of birth, medical record number and visit number.
- Your care provider is required to check your wristband before giving you medications, drawing blood or giving you blood products.
- You also may be asked to give your name to ensure accuracy.
Preventing Medical Errors
To prevent the wrong tests or procedures from being ordered, we take steps to verify who you are and what you are having done.
- Your doctor may ask questions to verify your identity and the type of procedure you are going to have.
- You may be asked to identify the site of your procedure.
- You will experience multiple caregivers verifying your identity before medications, procedures, consultations and discussions about your care. This may seem repetitive, but it is for your protection.
Medication Safety
Medication safety depends, in part, on close communication with your physician about supplements and drugs you may be taking and any allergies you have.
- Tell your care provider if you are using any herbal supplements, vitamins or other over-the-counter drugs.
- Tell your care provider if you have a food or drug allergy.
- Ask questions about your prescribed medications.
- Ask for a printed document of your medication to review the benefits and side effects.
- Tell your care provider if you have a problem with any medication, including unusual reactions or concerns about cost.
- Check with your care provider before changing how you take your medications.
- Never share your medication or use drugs prescribed to someone else.
- Store your medications safely away from children and pets.
Healthcare Provider Responsibilities
At Well Clinic, our healthcare providers are committed to the highest standards of care. Here's what you can expect from our team.
Our healthcare providers have the responsibility to:
- Communicate clearly with patient, including providing translation in the case of language barrier, or adaptation in the case of other disabilities.
- Inform patient about services that are available at the facility and eligibility based on health insurance or out-of-pocket cost.
- Inform patient whether proposed treatment or procedure is experimental or part of on-going research.
- Answer questions about proposed treatment or procedures. Provide information about potential risks, alternative treatment and procedures.
- Treat patient's human remains and tissue samples with care, consideration and dignity without prejudice to gender, religion, race, and ethnicity.
- Keep medical records of patients except when disclosure is by operation of Law or in the interest of public health.
- In the event of an emergency, provide immediate and sufficient intervention and care. Provider has a right to fees and should use legal means to recover same.
- Maintain records of personnel's current certificates and licenses to practice.
